Who We Are

Action Wellness Prison Medical Case Managers work with clients who are living with HIV to connect them to care. Clients are connected to healthcare, community resources, social support programs, and HIV education as they transition from jail back into the community.

What We Do:

The Philadelphia Linkage Program (PLP) serves people living with HIV while they are incarcerated and continues through their release from the Philadelphia Department of Prisons. We offer:

Re-entry Services

  • Medical case management
  • Discharge planning
  • Assistance in meeting probation terms
  • GED classes & twilight programs
  • Access to job skill training
  • Resumé writing

Why We Help

With our support, our clients stabilize their healthcare and medication regimen and make a connection to social services so that they have a better chance for a productive future. Clients are linked with community medical care and continue to receive services for up to 6 months after they re-enter their communities. Through this program, and many others at Action Wellness, we are creating the first AIDS-free generation and helping people living with HIV to achieve wellness.
