The Linkage and Engagement After Prison (LEAP) Program is a medical case management and outreach initiative for people with a diagnosis of Opioid Use Disorder (OUD). LEAP Case Managers provide valuable services to incarcerated and recently released individuals at The Philadelphia Department of Prisons (PDP).

What is the LEAP Program?
What do LEAP Case Managers do?
LEAP Case Managers (LCMs) meet with incarcerated individuals at PDP to assess their needs, build relationships, and create a discharge plan. Following release, LEAP Case Managers continue to engage and support them, ensuring that they are connected to drug treatment, necessary harm reduction, and/or social services. Case Managers continue to work with the client for three to six months post-release to promote a smooth transition back into the community.

- Medication for opioid use disorder (MOUD)
- Inpatient/outpatient drug and alcohol treatment
- Mental health treatment
- Primary and specialist medical care
- HIV testing
- Narcan training
- Syringe service programs
- Educational programs
- Vocation support and job training
- Food resources
- Transportation
- Vital documents
- Housing counseling
- Welfare/Social Security/Veterans benefits
- Court Advocacy
- Assistance in meeting probation requirements
Program Eligibility
The LEAP Program is open to individuals currently incarcerated at PDP who have a diagnosis of OUD and are preparing for release. Participants must be willing to engage in case management services, including the development of a personalized discharge and reentry plan.
