What is the Philadelphia Linkage Program?

The Philadelphia Linkage Program (PLP) is a medical case management initiative for those living with HIV and currently incarcerated or recently released from the Philadelphia Department of Prisons (PDP). Clients are connected to healthcare, community resources, social support programs, and HIV education as they transition from jail back into the community.

What do PLP Case Managers do?

PLP Case Managers meet with incarcerated individuals at PDP to assess their needs, build relationships, and create a discharge plan. Following release, PLP Case Managers continue to engage and support recently released individuals, ensuring that they are connected to HIV medical care and social services.

PLP Services include linkage to:

  • Medical care
  • Medication assistance & access
  • Health insurance
  • Medication for opioid use disorder (MOUD)
  • Inpatient/outpatient drug and alcohol treatment
  • Mental health treatment
  • Primary and specialist medical care
  • Educational programs
  • Vocation support and job training
  • Food resources
  • Transportation
  • Vital documents
  • Housing counseling
  • Welfare/Social Security/Veterans benefits
  • Court advocacy

Program Eligibility

PLP is available to individuals living with HIV who are currently incarcerated at, or have been recently released from PDP. Participation is voluntary and available regardless of insurance, income, or medical provider.
