The Perinatal Program provides services to pregnant persons living with HIV or substance use disorder. Staff are trained in the specific needs of pregnancy and are available for any individual, regardless of where they receive medical care.

What Do Perinatal Case Managers Do?
Perinatal Case Managers provide specialized support to pregnant individuals living with HIV or substance use disorder. They coordinate care, connect clients to essential resources, and offer guidance throughout pregnancy, ensuring both parents and babies receive comprehensive support.
Family & Perinatal Services include:
- Medical & medication adherence support
- Lab results monitoring for health risks for birthing person and child
- Safety and intimate partner violence screenings
- Linkage to public benefits & insurance
- Monthly Support Group
- Pantry (First Foods, Formula, clothing, diapers, Hygiene items)
- Lactation Support
- Referrals for baby equipment/clothing (Pump for chest/breast feeding, car seats, Pack & Plays)
- Education on formula feeding, co-sleeping and nutrition
- Prevention Counseling
- Linkage to pediatric care
- Postpartum education (safe sleeping, self-care, contraception)
- Risk reduction counseling
- Home Visitation Services
- Assistant Case Management Services (child care, assistance with skills of daily living and social support)
Program Eligibility
The Perinatal Program is open to persons who are pregnant and living with HIV or substance use disorder. Services are available regardless of age, income, insurance status, or where medical care is received.
