What is Medical Case Management?

Medical Case Management (MCM) is a collaborative process where professionals work one-on-one with individuals throughout their health journey. The goal is to link and maintain individuals in medical care and to make sure they are taking their prescribed medications.

What do Medical Case Managers do?

Medical Case Managers help individuals navigate complex systems to access essential services such as medical and behavioral healthcare, medications, health insurance, transportation, and other necessary resources.

Medical Case Management Services include:

  • Assessing unmet needs
  • Making a plan and coordinating services
  • Evaluating outcomes to ensure clients receive appropriate and effective services

This program serves individuals with HIV or at risk of acquiring HIV and their families.
