Opioid Care Outreach Specialist (LEAP Program)

Salary Range: $37,000 – $42,000 Annually

Position Title:  Care Outreach Specialist (LEAP)

Reports To:     Linkage and Engagement After Prison (LEAP) Case Manager

Department:    Direct Services

Status:             Full-time Monday through Friday 9am-5pm, Non-Exempt, Eligible for Benefits

Qualifications: High school graduate or GED equivalent with a stable work history. Solid written and oral communication skills are necessary. Good judgement, ability to be compassionate. Prior experience in the health or social work field is desired. Knowledge of community resources or experience dealing with individuals with substance use disorder and/or formerly incarcerated people is a plus. Some flexibility needed concerning work hours, occasional evenings and weekends are required for work events.

Basic Function: To support the client’s ability to manage life and activities of daily living, assist in maintaining contact with clients who are diagnosed with substance use disorder. Must understand and be willing to adhere to the philosophy and practice of harm reduction. Under the direction of the LEAPP Coordinator, provide support to clients in accessing benefits and entitlements. Assist and link clients with primary medical care and supportive social services.  Follow-up in the field on clients who have become lost to care (Clients who have not maintained contact with Case Manager or Primary Medical Care).

*Please note that while the majority of programs at Action Wellness serve clients living with HIV, this is a program that serves those living with opioid use dependence who are HIV negative.

Specific Responsibilities:

  • Accompany clients to medical and social service related appointments.
  • Assist case managers in relocating clients are lost to follow-up.
  • Document and monitor progress and participation of clients connected to services.
  • Attend and participate in all Action Wellness staff meetings.
  • Participate in training, supervision and staff development as required.
  • Maintain accurate records and documentation of services provided.
  • Provide information, education, and practical assistance to clients and members of their support system as required.
  • Other duties as required.
